Job summary

ProHealth Partners in Perrysburg, OH, provides home health therapy across Northwest and Southwest Ohio. We are seeking a licensed Occupational Therapist for full-time opportunities in the Tiffin area service territories.

As a therapist, you will evaluate patients in their homes, coordinate with the office and therapy team, and handle documentation and case conferences. Travel to patient homes is required to deliver quality care.

Qualifications

• Current Occupational Therapist Licensure.
• Bachelor's degree in Occupational Therapy.
• Ability to travel to patient's home or facility.

Responsibilities

• Communicating daily with office staff to accept new patients and complete payroll.
• Maintaining open communication with patients and home health agencies to ensure quality care.
• Facilitating coordination with Physical Therapist Assistant to manage patient care.
• Completing Evaluation, Plan of Care, Reassessment, and Discharge paperwork.
• Attending meetings and case conferences.
