Qualifications

• Current Physical Therapist licensure is required.

• Ability to travel to patients’ homes or facilities.
• Home healthcare experience preferred.
• Daily commute to patients’ homes and facilities is required.

Responsibilities

• Supervision of assistant therapy staff.
• Communicate daily with office staff to accept new patients, complete paperwork and payroll.
• Maintain open communication with patients and home health agencies to ensure quality care.
• Facilitate communication with Physical Therapist Assistant to manage patient care.
• Complete Evaluation, Plan of Care, Reassessment, and Discharge visits and related paperwork.
• Attend meetings and clinical case conferences.

Job Responsibilities:
