Qualifications

• Degree in Occupational therapy from an accredited school.
• Current Occupational Therapy Assistant License for the state of Ohio.
• Ability to travel to patient’s home or facility.

Responsibilities

• Communicating daily with office staff to accept new patients, complete paperwork and turn in payroll.
• Keeping open communication with patients and home health agencies to promote a positive and caring experience.
• Facilitating communication with Lead Occupational Therapist to manage patient care.
• Completion of in home therapy service, education to patients and caregivers and diligent detailed notes on all paperwork.
• Attend meetings (as needed with the Fremont therapy team).
