Job summary

Windsor Home Health is seeking a compassionate Occupational Therapist to join our Home Health team in Ohio. This full-time role offers a flexible schedule to deliver patient-centered OT services in patients' homes.

You will evaluate functional abilities, create individualized plans, educate patients and caregivers, and collaborate with a multidisciplinary team while maintaining thorough documentation and progress monitoring.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in the state of Ohio.
• Home health experience preferred, but not required.
• Strong communication and organizational skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Provide occupational therapy services in the home setting.
• Educate patients and caregivers on adaptive techniques and safety strategies.
