Job summary

Remedy Home Health Care is seeking a Per-Diem Occupational Therapist (OT) to deliver patient-centered care directly in patients’ homes across Ventura County, including Oxnard, Camarillo, Thousand Oaks, and Moorpark.

You will perform in-home evaluations, develop personalized care plans, and collaborate with nurses and physicians to improve ADLs, safety, and independence. Strong communication and reliable transportation are essential.

Qualifications

• Licensed Occupational Therapist in California.
• Graduate of an ACOTE-accredited OT program.
• Minimum 1 year of clinical OT experience (Home Health preferred).
• Familiar with electronic documentation systems (EMRs).
• Excellent communication and time management skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Perform in-home OT evaluations and reassessments.
• Create individualized care plans tailored to each patient’s goals.
• Collaborate with nurses, physical therapists, and physicians.
• Educate patients and caregivers on proper techniques and safety.
• Document patient progress in compliance with state and Medicare guidelines.

Key Responsibilities

• Regain independence through customized therapy plans

Your Day-to-Day Includes:
