Job summary

Remedy Home Health Care is seeking a licensed Occupational Therapist to deliver patient-centered home health services in Oxnard, CA. You will perform in-home evaluations, develop individualized plans, and coordinate with a clinical team to ensure safe, effective care.

The role offers per-visit compensation ($120–$140) with flexible scheduling and opportunities for professional growth, staying current with CEUs and training.

Qualifications

• Licensed Occupational Therapist in California.
• Graduate of an ACOTE-accredited OT program.
• Minimum 1 year of clinical OT experience (Home Health preferred).
• Familiar with EMR/documentation systems.

Responsibilities

• Perform in-home OT evaluations and reassessments.
• Create individualized care plans tailored to patient goals.
• Collaborate with nurses, PTs, and physicians.
• Educate patients and caregivers on proper techniques and safety.
• Document patient progress per state and Medicare guidelines.

Key Responsibilities

As a Home Health Occupational Therapist , you’ll help patients:

• Regain independence through customized therapy plans
• Improve functional abilities for activities of daily living (ADLs)
• Recover from injury, surgery, or hospitalization in the comfort of home
• Reduce fall risk and improve home safety

Your Day-to-Day Includes:

• Performing in-home OT evaluations and reassessments
• Creating individualized care plans tailored to each patient's goals
• Collaborating with nurses, physical therapists, and physicians
• Educating patients and caregivers on proper techniques and safety
• Documenting patient progress in compliance with state and Medicare guidelines

Qualifications

• Licensed Occupational Therapist in the state of California
• Graduate of an ACOTE-accredited OT program
• Minimum 1 year of clinical OT experience (Home Health experience preferred)
• Familiar with electronic documentation systems (EMRs)
• Excellent communication and time management skills
• Valid driver’s license, auto insurance, and reliable transportation
