Job summary

Remedy Home Health Care is seeking a Per-Diem Occupational Therapist to deliver patient-centered home health services across Ventura County. You will conduct in-home OT evaluations, design individualized plans, and collaborate with nurses and physicians to meet patient goals.

Licensed CA OT with ACOTE-accredited training and at least 1 year of clinical experience is required; EMR experience and reliable transportation are preferred.

Qualifications

• Licensed OT in California and ACOTE-accredited OT program.
• Minimum 1 year of clinical OT experience; Home Health preferred.
• Familiar with EMR documentation and state/Medicare guidelines.

Responsibilities

• Regain independence through customized therapy plans.
• Improve functional abilities for ADLs.
• Recover from injury, surgery, or hospitalization in the home.
• Reduce fall risk and improve home safety.
• Perform in-home evaluations and reassessments.
• Create individualized care plans and educate patients.

Key Responsibilities

Your Day-to-Day Includes:

• Performing in-home OT evaluations and reassessments
• Creating individualized care plans tailored to each patient’s goals
• Collaborating with nurses, physical therapists, and physicians
• Educating patients and caregivers on proper techniques and safety
• Documenting patient progress in compliance with state and Medicare guidelines

Qualifications

• Licensed Occupational Therapist in the state of California
• Graduate of an ACOTE-accredited OT program
• Minimum 1 year of clinical OT experience (Home Health experience preferred)
• Familiar with electronic documentation systems (EMRs)
• Excellent communication and time management skills
• Valid driver’s license, auto insurance, and reliable transportation
