Job summary
Remedy Home Health Care is seeking a licensed Occupational Therapist to deliver patient-centered home health services in Oxnard and surrounding Ventura County areas. The Per‑Diem OT will perform in-home evaluations, create customized care plans, and collaborate with nurses, physicians, and other therapists to restore independence and safety in patients' homes.

Qualifications
- Licensed Occupational Therapist in the state of California.
- Graduate of an ACOTE-accredited OT program.
- Minimum 1 year of clinical OT experience (Home Health experience preferred).
- Familiar with electronic documentation systems (EMRs).
- Excellent communication and time management skills.
- Valid driver’s license, auto insurance, and reliable transportation.
Responsibilities
- Perform in-home OT evaluations and reassessments.
- Create individualized care plans tailored to each patient’s goals.
- Collaborate with nurses, physical therapists, and physicians.
- Educate patients and caregivers on proper techniques and safety.
- Document patient progress in compliance with state and Medicare guidelines.
