Job summary

Nans Pointe Rehabilitation and Nursing in Suffolk, VA is seeking an Occupational Therapist to join our in-house therapy team. You will assess referred patients, develop and implement rehab plans, and work within a patient-centered care model.

The role requires graduation from an accredited OT program and a valid state license. Experience in long-term or geriatric care is preferred, with collaboration across disciplines and adherence to HIPAA and Residents' Rights.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities and rehab potential.
• Establish and administer a treatment program with goals according to patient capacity and tolerance.
• Set realistic and achievable goals for patients.
• Document and record patients’ condition.
• Educate patients and families in an appropriate therapy method.
• Perform evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborate with all disciplines to provide patient-centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA and Residents Rights compliance.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
