Job summary

Nans Pointe Rehabilitation and Nursing in Suffolk, Virginia seeks an Occupational Therapist to join our in-house therapy team. The role focuses on resident-centered rehabilitation with direct patient care and collaboration with a multidisciplinary team to deliver evidence-based therapy.

You will assess referred patients, develop treatment plans, educate patients and families, and document progress, while complying with confidentiality and regulatory standards.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to assess medical condition, functional capabilities, and rehab potential.
• Establish and administer a treatment program with goals based on patient capacity and physician direction.
• Set realistic goals and document patient progress and condition.
• Educate patients and families in appropriate therapy methods.
• Perform evaluations, progress notes, re-certification and discharge documents.
• Collaborate with all disciplines to provide patient-centered care.
• Perform duties as assigned by Rehabilitation Director.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
