Job summary

Nans Pointe Rehabilitation and Nursing is seeking an Occupational Therapist to join our in-house therapy team. You will assess patients, develop individualized treatment plans, and collaborate with a multidisciplinary team to deliver high-quality rehab services.

Ideal candidates have graduated from an accredited OT program, hold a valid state license, and have experience in long-term or geriatric care. We offer a supportive work environment and ongoing training.

Qualifications

• Must graduate from an accredited Occupational Therapist program.
• Must hold a valid state license and registration.
• Experience in long-term or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care to evaluate medical condition and functional status.
• Develop and administer treatment plans with physician guidance.
• Set realistic goals based on patient capacity.
• Document patient condition and progress weekly.
• Educate patients and families on therapy methods.
• Collaborate with interdisciplinary team to deliver patient-centered care.
• Complete discharge documentation and certifications as needed.
• Comply with confidentiality and HIPPA.

We are currently seeking an Occupational Therapist to join our in-house therapy

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
