Job summary

Nans Pointe Rehabilitation and Nursing is seeking a dedicated Physical Therapist (PT) to join our resident-centered team. Provide direct patient care, assess rehabilitation needs, and develop individualized treatment plans for geriatric patients in long-term care settings.

Responsibilities include documenting progress, coordinating with the care team, and ensuring compliance with HIPAA and residents' rights while delivering compassionate, evidence-based therapy.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.
• Experience with Case Mix Index and PPS scheduling is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and evaluate medical condition, function, and rehab potential.
• Establish and manage treatment programs with physician-guided goals.
• Set realistic, achievable goals for patients.
• Document patient condition and educate patients and families.
• Conduct evaluations, weekly progress notes, recertification, discharge docs.
• Collaborate with disciplines to deliver patient-centered care.
• Perform duties assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA, and residents' rights.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services.

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.

• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.

• Set realistic and achievable goals for their patients

• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.

• Evaluations, weekly progress notes, recertification and discharge documentations.

• Collaborates with all disciplines to provide best patient centered treatment and care.

• Perform all other duties assigned by Director of Rehabilitation

• Compliance with confidentiality, HIPPA and Federal Residents Rights.
