Job summary

Premier Home Health Providers Inc. seeks an Occupational Therapist to support patients in their homes on a per diem basis. You will assess functional abilities, plan personalized in-home therapy, and coach patients and families to restore independence and safety in daily activities.

The role requires CA OT licensure, NBCOT registration, CPR (AHA), and a minimum of 2 years of clinical OT experience. A valid driver's license and reliable vehicle are also required, with mileage reimbursement

Responsibilities

• Evaluate patients in their home to identify safety needs and functional goals.
• Develop and communicate personalized OT treatment plans to physicians and the care team.
• Teach and retrain daily living activities at home.
• Recommend adaptive equipment and educate on usage.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with the care team for seamless patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and home health aides as needed.
• Support discharge planning as patients transition from home health care.

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
