Job summary

Premier Home Health Providers Inc. is seeking a compassionate Occupational Therapist to deliver high-quality home-based care. This per diem role offers autonomy, patient-centered planning, and collaborative support from a family‑oriented team.

Responsibilities include evaluating home environments, creating treatment plans, training patients in daily living activities, and coordinating with physicians. Must have CA OT license and travel flexibility.

Qualifications

• Must hold an OT degree from an accredited program.
• Registered with NBCOT and active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver's license, insured vehicle, and ability to travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop personalized OT treatment plans and coordinate with physicians.
• Teach and retrain patients in daily living skills.
• Recommend adaptive devices and prosthetics/orthotics as needed.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with care team for seamless patient-centered care.
• Educate family on treatment approach.
• Supervise OTAs and oversee home health aide care when OT is sole skilled service.
• Support discharge planning during transitions home.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
