Job summary

Premier Home Health Providers Inc. is seeking a licensed Occupational Therapist to deliver per diem home health services across patient homes.

You will evaluate functional abilities, create treatment plans, and educate patients and families while coordinating with physicians and the care team. Travel to homes is required, and a flexible schedule is offered with potential to convert to full-time based on performance.

Qualifications

• Graduate of an OT curriculum accredited by the AMA/AOTA
• Registered with the National Registration Examination of AOTA
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license and insured vehicle, travel to patient homes
• Strong communication and organizational skills

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living activities
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the treatment approach
• Supervise OT Assistants and oversee home health aides when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $110 - $130 per visit.

Responsibilities

• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices

• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
