Job summary

Total Care Therapy is seeking a dedicated Occupational Therapist in Schererville, IN to evaluate residents and build individualized plans to enhance independence in daily activities. You will implement therapies addressing ADLs, cognition, and safety, while collaborating with an interdisciplinary team across assisted living and independent living communities.

You will supervise COTAs, document progress in EMR, and educate residents and staff on adaptive strategies.

Qualifications

• CurrentState OT License required.
• CPR/BLS Certification required.
• Valid driver’s license with reliable transportation.
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required.

Responsibilities

• Evaluate residents and develop individualized OT plans of care.
• Provide interventions to improve ADLs, IADLs, cognition, balance, and safety.
• Assess adaptive equipment needs and recommend environmental modifications.
• Supervise COTAs per state regulations and ensure proper documentation in EMR.
• Educate residents, caregivers, and staff on home safety and fall prevention.
• Collaborate with interdisciplinary team for coordinated patient care.

About the Role

As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess patient progress and modify treatment plans as appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete timely and accurate documentation using the electronic medical record (EMR).
• Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.
• Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

• Current State Occupational Therapist (OT) License
• Current CPR/BLS Certification
• Valid driver's license, reliable transportation, and auto insurance
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
