Total Care Therapy in Mayfield, OH is seeking an Occupational Therapist for a part-time role in assisted living and independent living communities. You will evaluate residents and create individualized treatment plans to improve daily living activities, cognition, and safety, while supporting a therapist-owned culture.

Qualifications

• Current State OT license to practice.
• CPR/BLS certification current.
• Valid driver's license and reliable transportation.
• Experience in geriatrics, home health, or assisted living preferred but not required.

Responsabilités

• Evaluate residents and establish individualized occupational therapy plans of care.
• Provide skilled OT interventions to improve ADLs, IADLs, cognition, balance, and independence.
• Assess and recommend adaptive equipment and environmental modifications.
• Reassess progress and modify treatment plans as appropriate.
• Supervise COTAs in accordance with state regulations.
• Complete timely documentation using the EMR.
• Educate residents, caregivers, and staff on safety techniques and energy conservation.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.

Salary Range: $60.00 To $70.00 Hourly

Setting: Assisted Living and Independent Living Communities
Available Locations:

Mayfield

Scheduling: Monday-Friday ⎸Part-Time (Weekends available but not required)

Compensation: Per-Visit (based on years of experience)

• Evaluations: $60-$70 per visit
• Reassessments: $45-$55 per visit

About the Role
As an Occupational Therapist, you will evaluate residents and develop individualized treatment plans that improve independence with activities of daily living, upper extremity function, cognition, and overall safety.

Responsibilities

Evaluate residents and establish individualized occupational therapy plans of care.

Provide skilled occupational therapy interventions to improve ADLs, IADLs, upper extremity function, cognition, balance, coordination, and functional independence.

Assess and recommend adaptive equipment and environmental modifications.

Reassess patient progress and modify treatment plans as appropriate.

Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.

Complete timely and accurate documentation using the electronic medical record (EMR).

Educate residents, caregivers, and facility staff on adaptive techniques, home safety, fall prevention, and energy conservation strategies.

Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.

Qualifications

Current State Occupational Therapist (OT) License

Current CPR/BLS Certification

Valid driver's license, reliable transportation, and auto insurance

Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required
