Job summary

Santa Clarita Post Acute is seeking a dedicated Occupational Therapist to join our in-house rehab team. You will evaluate and treat residents with physical, cognitive, and psychosocial impairments to maximize independence and quality of life in a skilled nursing setting.

Qualifications

• Current license to practice as an Occupational Therapist in California.
• Minimum of 2 years of experience in a healthcare setting, preferably skilled nursing.
• Knowledge of OT practice standards and regulations.
• Proficiency with electronic medical records and healthcare software.

Responsibilities

• Evaluate residents' abilities and functional limitations using standardized assessments.
• Develop and implement individualized treatment plans based on assessments and goals.
• Provide skilled OT interventions including ADL training, therapeutic exercise, cognitive retraining, and environmental modifications.
• Monitor and document progress; adjust plans as necessary.
• Coordinate care with physicians, nurses, PTs, and SLPs; educate residents and families on goals and home programs.
• Maintain accurate documentation and participate in team meetings and quality initiatives.

Santa Clarita Post Acute is currently seeking both full time and per diem Occupational Therapists to join our In-House Team and assume a rewarding position in patient rehabilitation. We value our skilled rehab professionals who can guide patients through personalized therapy and provide ongoing support.

As the Occupational Therapist, you will be responsible for evaluating and treating residents with physical, cognitive, and psychosocial impairments to help them achieve their highest level of independence and quality of life.

Responsibilities

• Evaluate residents' physical, cognitive, and psychosocial abilities and functional limitations using standardized assessments
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
• Provide skilled occupational therapy interventions, including therapeutic exercise, activities of daily living training, cognitive retraining, and environmental modifications
• Monitor and document resident progress and adjust treatment plans as necessary
• Communicate with interdisciplinary team members, including physicians, nurses, physical therapists, and speech therapists, to coordinate care and ensure optimal outcomes for residents
• Educate residents and family members on occupational therapy goals, treatment plans, and home exercise programs
• Maintain accurate and timely documentation of occupational therapy services provided
• Participate in team meetings, in-service education, and quality improvement initiatives
• Maintain current knowledge of occupational therapy practice and relevant regulations

Qualifications

• Current license to practice as an Occupational Therapist in the state of California
• Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
• Knowledge of current occupational therapy practice standards and regulations
• Proficient in using electronic medical records and other healthcare software
