Job summary

Midtown Oaks Post Acute in Sacramento, CA, seeks a full-time Occupational Therapist to join our skilled nursing team. You will evaluate residents, develop individualized treatment plans and provide skilled OT services to improve function and independence.

You will collaborate with physicians, nurses, therapists and families to optimize outcomes, educate on treatment plans and devices, and maintain compliant documentation while supporting resident-centered care.

Qualifications

• Active California Occupational Therapist License
• Experience in geriatric/ SNF settings is preferred
• Strong assessment, treatment planning and documentation skills

Responsibilities

• Provide skilled occupational therapy services to improve function, mobility and independence
• Evaluate residents and develop individualized treatment plans
• Assess activities of daily living (ADLs) and implement therapeutic interventions
• Collaborate with physicians, nursing staff, therapists and families to optimize outcomes
• Educate residents and caregivers on plans, equipment and recovery strategies
• Maintain accurate documentation and ensure regulatory compliance
