Job summary

PACS Group's Citrus Heights Post Acute is seeking a dedicated Occupational Therapist to join our rehab team in Citrus Heights, CA. You will evaluate patients, create individualized plans, and deliver therapy to improve independence and daily living skills within a supportive post-acute setting.

You will supervise COTAs and collaborate with physicians, nurses, and staff to optimize patient outcomes. A CA OT license and a Bachelor's in OT are required; a Master's/Doctorate is preferred.

Qualifications

• Current and active California Occupational Therapist License required.
• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate in Occupational Therapy preferred.
• Experience with PointClickCare and Casamba preferred.
• Strong clinical assessment and treatment planning skills.

Responsibilities

• Evaluate patients within required regulatory timeframes following physician referral.
• Develop individualized treatment plans and establish measurable goals.
• Provide occupational therapy services in accordance with physician orders and professional standards.
• Supervise COTAs, aides, and students as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members regarding patient progress and care plans.
• Participate in care conferences, rehab meetings, and discharge planning.
• Educate patients, families, caregivers, and staff on treatment plans and therapy techniques.
• Recommend adaptive equipment, assistive devices, and durable medical equipment as needed.
• Complete accurate and timely documentation in compliance with guidelines.
• Maintain therapy equipment and report maintenance or repair needs.
