Job summary

Trilogy Home Healthcare, a Humana company, is seeking a dedicated Occupational Therapist (OT) to provide home health visits in Bay County, FL on a PRN/per diem basis. The role requires Florida OT licensure, an accredited OT degree, and at least 1 year of OT experience, with home health experience preferred.

You'll collaborate with doctors and interdisciplinary teams to help patients regain mobility and independence, educate families, and ensure safe home environments.

Qualifications

• Graduate of an accredited OT program (AOTA-approved).
• Active OT license in Florida.
• 1+ years of OT experience; Home Health experience preferred.

Responsibilities

• Deliver compassionate, high-quality home health OT services.
• Collaborate with doctors, Clinical Team Managers, and interdisciplinary teams.
• Help patients regain mobility, strength, and independence after illness or surgery.

What You Bring:

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved)
• Active OT license in Florida
• Valid Florida drivers license, active vehicle insurance, and clear MVR
• 1+ years of experience as an OT required, Home Health experience preferred
• Strong initiative and independent judgement
