Occupational Therapist – Full Time – Olympia Transitional Care – Olympia, WA

Olympia Transitional Care is a skilled nursing facility in Olympia, WA, offering long‑term care, post‑acute rehab, and geriatric outpatient therapy services. We are an in‑house therapy team with no contract company, providing job stability, mentorship, leadership development, and a flexible schedule with competitive benefits.

Responsibilities

• Effectively screen and evaluate patients with cognitive and/or ADL or physical functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment to patients, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Administer various procedures as part of the rehabilitation plan.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness and modify services based on patient evaluation.
• Refer patients as needed based on outcomes.
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to the physical disorders.
• Effectively communicate with the supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provide a comprehensive discharge summary of services per regulatory and clinical requirements.
