Position Overview

Pacific Care Center in Hoquiam, WA is hiring a full‑time Occupational Therapist to deliver high‑quality rehabilitative care and contribute to program and leadership development.

Qualifications

• Licensed Occupational Therapist.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care in compliance with regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment interventions, including ADL retraining, therapeutic activities, exercises, modalities, and use of supportive devices.
• Administer various rehabilitation procedures and adjust treatments based on patient progress and clinical record updates.
• Evaluate treatment outcomes, determine effectiveness, and modify services accordingly.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Communicate with supervisors and health team members regarding patient progress, barriers, and treatment plans.
• Prepare comprehensive discharge summaries per regulatory requirements.
