Responsibilities

• Provide occupational therapy evaluation and treatment to patients in their homes or alternate community settings.
• Assess and evaluate clients for occupational therapy needs and develop, implement, and adjust plans of action in collaboration with physicians.
• Document changes in client condition and update care plans and outcomes accordingly.
• Identify counseling and education needs for patients, caregivers, and families and plan teaching strategies.
• Monitor quality and appropriateness of services provided.
• Participate in VNAHG’s QAPI and in-service programs and adhere to infection control policies.

Position Summary:

Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings.

Principal Responsibilities:

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician
• Document all changes in client’s condition and updates care plans and outcomes accordingly
• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies
• Monitor quality and appropriateness of services provided
• Participate in VNAHG’s QAPI and in-service programs
• Adhere to agency infection control and safety policies
• Adhere to the organization’s policy in regard to absenteeism and appearance

Job Competencies & Minimum Qualifications

• Occupational Therapy Licensure in NJ

• Valid NJ driver’s license

Working Conditions/Physical Demand

Working indoors/outdoors, driving, lifting, sitting, standing, walking, fine motor hand movement, use of phone and tablet for documentation

In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000.
