Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to provide home health services across Nebraska. The OT will perform clinical assessments, develop and adjust a Plan of Care, and document progress notes under the physician’s plan of care.

Responsibilities include identifying patient needs, coordinating with the care team, and ensuring infection control and safety in the patient’s home environment. Two years of OT experience and CPR/Hepatitis profiles are required.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• CPR certification and Hepatitis profile current.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment of patients in home health.
• Identify patient needs and appropriateness of care.
• Document, prepare, and complete a Plan of Care and progress notes.
• Revise the Plan of Care based on ongoing assessment.
• Apply infection control and universal precautions in care.
• Coordinate care and communicate with the clinical team.
• Assess home environment components affecting occupation.
