Job summary

Angels Care Home Health in Creston, IA is seeking an Occupational Therapist to join our patient-focused team. You will perform clinical assessments, determine patient needs, and develop and revise the Plan of Care to support ongoing recovery at home.

Under physician direction, you will document progress notes, apply infection control practices, and collaborate with care teams to ensure services align with each patient’s goals and the agency's standards for quality home health care.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment and determine patient needs.
• Identify eligibility of the Plan of Care and revise as needed.
• Document, prepare, and complete the Plan of Care and progress notes.
• Apply infection control and universal precautions in patient care.
• Collaborate with team and coordinate care with the physician's plan of care.
• Assess the patient's occupational environment including physical, social, and cultural factors.
