Job summary

PRN Home Health & Therapy is seeking an Occupational Therapist to join our growing home health team in Monroe/South Madison. This 0.6-1.0 FTE role offers flexible scheduling and autonomy to deliver meaningful patient care in home settings with occasional SNF visits.

Develop individualized care plans, supervise COTAs, and collaborate with patients, caregivers, and the clinical team to maximize independence for older adults.

Responsibilities

• Work independently while maintaining strong communication with the clinical team.
• Review and respond to patient referrals promptly.
• Collaborate with patients, caregivers, and interdisciplinary team to develop care plans.
• Create treatment plans with measurable, patient-centered goals.
• Educate patients and caregivers to support outcomes and independence.
• Provide guidance and supervision to COTAs.
• Assess and recommend DME including wheelchairs and adaptive equipment.
• Partner with healthcare professionals to ensure high-quality care.
• Perform additional duties as assigned.

PRN Home Health & Therapy is seeking a dedicated Occupational Therapist to join our growing team.

This position provides the freedom to manage your own schedule while making a lasting impact on patients throughout Monroe/South Madison.

In this role, you'll primarily provide care in a home health setting, with occasional visits to skilled nursing facilities. Whether you're an experienced clinician or a new graduate eager to begin a rewarding career in home health, this position offers an excellent opportunity to grow and succeed.

Key Responsibilities

• Work independently while maintaining strong communication with the clinical team.
• Review and respond to patient referrals in a timely manner.
• Collaborate with patients, caregivers, and interdisciplinary team members to develop individualized care plans.
• Create and implement treatment plans with measurable, patient-centered functional goals.
• Educate patients and caregivers to support successful outcomes and maximize independence.
• Provide guidance, direction, and supervision to Certified Occupational Therapy Assistants (COTAs).
• Assess and recommend durable medical equipment (DME), including wheelchairs, adaptive equipment, and other assistive devices.
• Partner with healthcare professionals to ensure coordinated, high-quality patient care.
• Perform additional duties as assigned.
