Job summary

Best Home Health Providers Inc. is seeking an Occupational Therapist to support patients in their homes across the Bay Area on a per diem basis. The OT will evaluate functional abilities, create and communicate treatment plans, and retrain patients in daily living activities.

The role requires an accredited OT degree, California license, NBCOT, CPR, and at least two years of clinical OT experience. A valid driver’s license and reliable vehicle for home visits are essential.

Qualifications

• OT degree from accredited program
• California OT license
• NBCOT/registered with AOTA
• CPR certification (AHA)
• Minimum 2 years of clinical OT experience
• Valid driver’s license and insured vehicle, travel to patient homes

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living activities
• Instruct patients on adaptive equipment and splints
• Maintain accurate clinical records and report changes in patient condition
• Coordinate with the care team for seamless patient-centered care
• Educate family members and caregivers on treatment approaches
• Supervise OT assistants and oversee home health aide care when OT is sole skilled service
• Support discharge planning as patients transition home

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
