Job summary

Vasona Creek Healthcare in Los Gatos, CA, is hiring an Occupational Therapist (OTR) to guide patients through rehabilitation after events such as joint replacements and strokes. The role offers opportunities across full-time, part-time, and PRN schedules with a supportive, multidisciplinary team and state-of-the-art facilities.

As an OTR, you will develop individualized treatment plans, supervise therapy assistants, and participate in care conferences while educating staff and families.

Responsibilities

• Evaluate patients and develop individualized treatment plans in accordance with physician orders.
• Provide skilled occupational therapy services and supervise Occupational Therapy Assistants in accordance with the California Occupational Therapy Practice Act.
• Document evaluations, progress notes, treatment sessions, and other required information accurately and timely.
• Collaborate with nursing, therapy, physicians, supervisors, and other interdisciplinary team members.
• Participate in patient care conferences, utilization review meetings, and rehabilitation conferences.
• Educate nursing staff, patients, and families on restorative programs, caregiver techniques, and strategies to support continued progress.
• Recommend and assist with obtaining appropriate durable medical equipment (DME) to promote safety and independence.
• Participate in discharge planning to support a safe and successful transition to the next level of care.
• Assist with facility in-service training and staff education programs.
• Monitor therapy equipment and promptly report maintenance or safety concerns.

Your Role as an Occupational Therapist
