Vasona Creek Healthcare in Los Gatos, CA is seeking a dedicated Occupational Therapist (OTR) to join our Short-Term Rehabilitation team. You will evaluate patients, develop individualized plans, and collaborate with a multidisciplinary team to restore function and independence.

Qualifications

• Current California Occupational Therapist (OTR) license or eligibility.
• Strong clinical assessment and treatment skills.

• Strong organizational and documentation skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans per physician orders.
• Provide skilled occupational therapy services and supervise OTAs per California OT Practice Act.
• Document evaluations, progress notes, and treatment sessions accurately and timely.
• Collaborate with nursing, therapy, physicians, and supervisors.
• Participate in care conferences and rehabilitation conferences.
• Educate staff, patients, and families on restorative programs and caregiver techniques.
• Assist with procuring appropriate DME for safety and independence.
• Participate in discharge planning for smooth transitions.
• Support in-service training and staff education programs.
• Monitor therapy equipment and report safety concerns.
