Signature Healthcare at Home seeks a compassionate Occupational Therapist to help patients regain independence in home settings along the Oregon Coast communities. You’ll perform comprehensive evaluations, design tailored treatment plans, and guide patients and caregivers in daily activities and adaptive strategies.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Oregon OT license.
• Current CPR certification or ability to obtain.
• Valid driver’s license, reliable transportation, and automobile insurance.

Responsibilities

• Evaluate patients’ functional abilities, mobility, safety, and home environments.
• Develop individualized treatment plans focused on maximizing independence and quality of life.
• Provide therapeutic interventions to improve ADLs and IADLs.
• Assess and recommend adaptive equipment, home modifications, and safety strategies.
• Train patients, caregivers, and families on home management techniques.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals.
• Complete accurate clinical documentation in accordance with agency standards.

What You’ll Do

• Evaluate patients’ functional abilities, mobility, safety awareness, and home environments.
• Develop individualized treatment plans focused on maximizing independence and quality of life.
• Provide therapeutic interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
• Assess and recommend adaptive equipment, home modifications, and safety strategies.
• Train patients, caregivers, and families on techniques that support successful home management.
• Collaborate with physicians, nurses, physical therapists, speech therapists, and other healthcare professionals.
• Complete accurate and timely clinical documentation in accordance with agency standards.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Oregon Occupational Therapist license.
• Current CPR certification or ability to obtain.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Home Health experience preferred but not required.
• Strong clinical judgment, organizational skills, and communication abilities.
