Job summary

Signature Healthcare at Home is seeking a Home Health Occupational Therapist (OT) to provide personalized in-home therapy across the Oregon Coast communities, including Lincoln City, Newport, Tillamook, and Florence. You will assess patients at home, develop treatment plans, and support independence in ADLs and IADLs, collaborating with a multidisciplinary team.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Oregon OT license.
• CPR certification or ability to obtain.

Responsibilities

• Evaluate patients' functional abilities, mobility, safety, and home environments.
• Develop individualized treatment plans to maximize independence and quality of life.
• Provide therapeutic interventions to improve ADLs and IADLs.
• Assess andRecommend adaptive equipment, home modifications, and safety strategies.
• Train patients, caregivers, and families on home management techniques.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals.
• Complete accurate and timely clinical documentation per agency standards.
