Trinity Healthcare Talent Services, LLC seeks an Occupational Therapist for a 110-bed SNF in Lebanon, NH. The role offers $37–$39.50 per hour and a 35-hour week with Tue-Sat or Sun-Thurs schedules, no weekend rotation. Excellent patient-centered care and collaboration with a multidisciplinary team are essential.

You will evaluate, plan, and implement therapy, educate families, and mentor OTAs while staying current with OT advances to improve residents’ independence and quality of life.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy.
• Current state license to practice as an Occupational Therapist.
• Minimum of 2 years of experience in a SNF or similar setting preferred.

Responsibilities

• Conduct thorough evaluations of patients' physical, cognitive, and emotional abilities.
• Develop individualized treatment plans to address patients' specific needs and goals.
• Implement treatment plans through a variety of therapeutic activities and exercises.
• Monitor and document patients' progress and adjust treatment plans as needed.
• Educate patients and their families on adaptive techniques and equipment to improve daily functioning.
• Collaborate with other healthcare professionals to ensure comprehensive care for patients.
• Provide guidance and support to Occupational Therapy Assistants and other staff members.
• Stay up-to-date with the latest advancements and techniques in Occupational Therapy to provide the best possible care for patients.
