Job summary

Trinity Healthcare Talent Services is seeking an Occupational Therapist for a 110-bed Skilled Nursing Facility in Lebanon, NH. You will evaluate patients, develop and implement treatment plans, and work closely with a multidisciplinary team to restore independence and improve quality of life.

Expect a 35-hour week, with Tue-Sat or Sun-Thurs shifts and no weekend rotation, and a competitive hourly rate of $37–$39.50, reflecting local market norms. Licensure and experience in SNF are preferred.

Qualifications

• Bachelor's or Master's degree in Occupational Therapy.
• Current state license to practice as an Occupational Therapist.
• Minimum of 2 years of SNF or similar setting preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Conduct thorough evaluations of patients' physical, cognitive, and emotional abilities.
• Develop individualized treatment plans to address patients' needs and goals.
• Implement treatment plans through therapeutic activities and exercises.
• Monitor and document progress and adjust plans as needed.
• Educate patients and families on adaptive techniques and equipment.
• Collaborate with other healthcare professionals to ensure comprehensive care.
• Provide guidance and support to Occupational Therapy Assistants and staff.
• Stay up-to-date with advancements in Occupational Therapy.
