Chautauqua Nursing and Rehabilitation Center in Dunkirk, NY is seeking an Occupational Therapist (OT) to join its rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide therapy to help residents regain independence and maximize function.

Responsibilities include establishing goals, supervising COTAs, documenting care plans, and collaborating with physicians and staff to support resident outcomes.

Qualifications

• Master's degree in Occupational Therapy from an accredited program.
• Valid New York State OT license.
• Newly licensed OT's encouraged to apply.
• Skilled nursing facility experience preferred.
• Strong clinical, documentation, communication, and problem-solving skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.
• Commitment to resident-centered care and rehabilitation excellence.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapy focused on improving functional abilities, ADLs, mobility, and quality of life.
• Establish short- and long-term rehabilitation goals and monitor resident progress.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations.
• Complete all required documentation, assessments, care plans, and billing records accurately and timely.
• Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs.
• Recommend and coordinate adaptive equipment and durable medical equipment (DME) needs.
• Participate in care conferences, discharge planning, staff education, and quality improvement initiatives.
• Perform additional duties as assigned.

REQUIREMENTS:
