AllStar Therapies is seeking a compassionate Home Health Occupational Therapist (OT) to deliver in-home, one-on-one therapy, helping patients regain independence and remain safely at home. You will evaluate patients, develop plans, and coordinate with physicians and care teams.

The role emphasizes independence, strong communication, accurate documentation, and flexible scheduling. Requirements include OT license in the state, accredited OT program, valid driver's license, and CPR preferred.

Qualifications

• Current OT license in the state of practice.
• Graduate of an accredited Occupational Therapy program.
• Home health experience preferred, but not required.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while managing a flexible daily schedule.
• Excellent communication, documentation, and organizational skills.
• CPR certification preferred.

Responsabilités

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled occupational therapy services in accordance with physician orders and the established plan of care.
• Assess and improve patients' ability to perform ADLs and IADLs.
• Recommend adaptive equipment, home modifications, and safety strategies.
• Educate patients, families, and caregivers on treatment plans and home exercise programs.
• Complete timely and accurate documentation in accordance with agency, state, federal, and Medicare guidelines.
• Communicate effectively with physicians, nurses, case managers, and the interdisciplinary care team.
• Participate in care conferences and discharge planning as needed.

AllStar Therapies is seeking a compassionate and dedicated Home Health Occupational Therapist (OT) to provide exceptional in-home therapy services. This rewarding position offers the opportunity to help patients regain independence and safely remain in their homes while delivering one-on-one, patient-centered care.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans

• Provide skilled occupational therapy services in accordance with physician orders and the established plan of care
