AllStar Therapies is seeking a compassionate and motivated PRN Occupational Therapist (OT) to provide skilled nursing therapy services. You will evaluate residents, develop individualized treatment plans, and support ADL independence in a compassionate, patient-centered manner.

You will collaborate with physicians, nursing staff, and families to ensure high-quality care, while documenting progress and discharge summaries.

Qualifications

• Current OT license in state of practice.
• Graduate of an accredited OT program.
• Previous SNF/rehab experience preferred but not required.
• Strong communication and organizational skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• CPR certification preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans

• Provide occupational therapy services in accordance with physician orders

• Help residents improve their ability to perform activities of daily living

• Document evaluations, treatments, progress notes, and discharge summaries accurately and timely

• Collaborate with physicians, nursing staff, rehabilitation team, residents, and families

• Participate in interdisciplinary care planning

• Ensure compliance with all state, federal, and company regulations

Qualifications

• Current Occupational Therapist (OT) license in the state of practice

• Graduate of an accredited Occupational Therapy program

• Previous experience in skilled nursing or rehabilitation is preferred but not required

• Strong communication and organizational skills

• Ability to work independently while collaborating with an interdisciplinary team

• CPR certification preferred
