Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our growing team. You will evaluate and treat patients in a SNF setting, following an individualized plan of care and collaborating with facility staff to provide excellent care.

Responsibilities include documenting evaluations and progress, re-evaluations, discharges and daily notes, while supervising Occupational Therapy Assistants and educating patients on outcomes and progress.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Supervising Occupational Therapy Assistants.
• Timely and accurate documentation including evaluations and progress notes.
• Collaborate with facility staff to ensure excellent care for your patients.
