Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our dynamic SNF team. The OT will evaluate and treat patients in a SNF setting, developing individualized plans of care and measurable goals. Collaboration with facility staff and timely documentation are essential, along with supervising Occupational Therapy Assistants.

Qualifications

• Licensed OT in Illinois.
• Experience in SNF settings preferred.
• Ability to supervise OTAs.

As an Enhance Rehabilitation Occupational Therapist, you will be a part of growing team. In this role, you will evaluate and treat patients in a SNF setting following an individualized plan of care.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
