Job summary

Enhance Rehabilitation is seeking a qualified Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to ensure high-quality therapy services.

You will document evaluations, progress notes, discharges, and treatment plans, and supervise Occupational Therapy Assistants as needed. Excellent patient communication and outcomes-focused care are expected.

Responsibilities

• Evaluating new patients and developing plans of care with measurable goals.
• Treating patients and educating them on injury, progress and outcomes.
• Re-evaluating progress, revising plans of care and resetting goals.
• Completing timely documentation: evaluations, re-evaluations, discharges and daily progress notes.
• Supervising Occupational Therapy Assistants and collaborating with facility staff to ensure excellent patient care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
• Supervising Occupational Therapy Assistants.
