Monument Ridge Health & Rehab in Grand Junction, CO is seeking a PRN Occupational Therapist for our in-house rehab program. The role involves screening, evaluation, and delivering skilled therapy to geriatric patients in a post-acute setting.

Licensed Occupational Therapists of all experience levels are welcome to apply. This position supports our goal of providing high-quality patient care within a supportive team environment.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, developing care plans.
• Deliver rehabilitative and medically necessary therapies.
• Assist with transfers and lifting as part of patient care.
• Administer procedures as part of the rehabilitation plan, including exercises and modalities.
• Provide counseling to patients and families regarding physical disorders.
• Prepare discharge summaries complying with regulatory requirements.

OCCUPATIONAL THERAPIST – PRN MONUMENT RIDGE HEALTH & REHAB – GRAND JUNCTION, CO Join our team and start making a difference! Monument Ridge Health & Rehab in Grand Junction, CO, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated PRN Occupational Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications:

• Occupational Therapist license is required.
• Open to OTs at all experience levels. New Grads are welcome!
