Delta Health and Rehabilitation in Delta, CO is seeking a dedicated Occupational Therapist to join our in-house team as a full-time therapist. You will screen and evaluate patients, design care plans and deliver rehabilitative interventions to improve physical functioning and wound care as needed.

We support strong orientation, ongoing training, and CAPLICO values in daily practice, with opportunities to contribute to discharge planning and cross-team collaboration.

Qualifications

• OT license is required.
• Open to OTs at all experience levels. New Grads welcome.
• Strong orientation, training and ongoing support are provided.

Responsabilités

• Screen and evaluate patients with physical functioning disorders and/or wound care.
• Develop care plans and deliver rehabilitative, skilled, and medically necessary treatment.
• Assist with regular standing/walking, lifting, transferring and repositioning as needed.
• Administer procedures as part of the rehabilitation plan, including manual techniques and therapeutic exercises.
• Provide consultation to patients, families and care providers regarding physical disorders.
• Generate discharge summaries in compliance with regulatory requirements.

We are excited to announce our growth in Colorado and are currently seeking passionate Occupational Therapists to join our in-house crew at Delta Health and Rehabilitation. As a therapy professional on our team, you will have the freedom to create and implement programs to best meet the needs of your patients. Our therapy team expansion will be a sharp focus, and you will receive strong orientation, training and on-going support as we grow!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Occupational Therapist license is required.
