Job summary

Diamond Care Group is seeking an experienced Occupational Therapist to provide home health services focused on improving patient independence and safety in the home. The role emphasizes in-home evaluations, customized treatment plans, and collaboration with a care team.

Qualified candidates must have an accredited OT program background and an active Florida OT license, with home health experience preferred. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited Occupational Therapy program and holds an active Florida OT license.
• Home health experience is preferred and OASIS experience is strongly preferred.
• Demonstrated strong documentation and time management skills.
• Valid driver’s license and reliable transportation are required.

Responsibilities
