Lake Eustis Healthcare & Rehabilitation is seeking a compassionate and experienced Occupational Therapist to join our rehabilitation team full time. In this role, you’ll provide resident-centered therapy, support recovery goals, and contribute to a positive, collaborative care environment.
Collaborate with nursing, administration, and other departments to provide effective therapy services.
Develop, implement, and maintain individualized occupational therapy care plans.
Conduct resident evaluations, progress reviews, care planning, and discharge coordination.
Maintain accurate documentation of therapy sessions, resident progress, and treatment outcomes.
Ensure therapy practices align with current clinical standards and regulatory requirements.
Provide guidance and support to therapy staff in collaboration with the Director of Rehabilitation.
Participate in quality assurance and performance improvement initiatives.
Communicate effectively with residents, families, and interdisciplinary team members.
Complete administrative duties, including documentation, reports, and billing accurately.
Active and valid Florida Occupational Therapist (OT) license.
Minimum 1 year of experience in skilled nursing or long-term care preferred.
Strong knowledge of therapy regulations, documentation standards, and rehabilitation best practices.
Excellent communication, organization, and clinical reasoning skills.
