Job summary

H2 Health is seeking an Occupational Therapist to deliver 1:1 care in a Senior Living community. You will help residents maintain independence and quality of life through goal-driven interventions, while coordinating with affiliated home health partners to ensure seamless transitions.

Qualifications

• Education: A graduate of an accredited Occupational therapy program.
• Experience: Prior experience is preferred, but new graduates are welcome!
• Qualifications: An active or eligible state license to practice

As an Occupational Therapist on our team, you'll help seniors maintain independence, confidence, and quality of life through meaningful, goal-driven interventions. You'll also play a vital role in our care continuity approach, collaborating with our home health partners within the same communities to ensure consistency and seamless transitions for both patients and staff.
