Qualifications

Are you looking to provide 1:1 care while seeing all your patients under one roof? H2 Health Prime Living has the pleasure to provide care to residents within luxury a Senior Living community in Sebastian!

As an Occupational Therapist on our team, you'll help seniors maintain independence, confidence, and quality of life through meaningful, goal-driven interventions. You'll also play a vital role in our care continuity approach, collaborating with our home health partners within the same communities to ensure consistency and seamless transitions for both patients and staff.
