Concierge Home Care in Crestview, FL is seeking a Home Health Occupational Therapist (OT) to deliver patient-centered care in the home setting. This PRN role offers a flexible schedule, autonomy, and 1:1 patient interactions with earning potential per visit.

Qualifications

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license with reliable transportation.
• CPR certification.

• Home health experience preferred, but not required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Perform home safety assessments and recommend modifications.
• Provide therapy to improve ADLs, functional mobility, and independence.
• Educate patients and caregivers on adaptive techniques and equipment.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Certified Occupational Therapy Assistants (COTAs).
• Complete documentation using streamlined technology.
