Excelcare in Manalapan, NJ is seeking a licensed Occupational Therapist to assess and treat patients in a rehabilitation setting. You will work under physician direction to develop individualized plans, document progress, and collaborate with a multidisciplinary team.

The role emphasizes patient education, care for geriatric populations, and compliance with HIPAA and residents’ rights in a long-term care environment.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.

Responsabilités

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, and rehabilitation potential.
• Establish and administer a treatment program with goals per patient capacity under physician direction.
• Set realistic and achievable treatment goals.
• Document and record patients’ condition.
• Educate patients and families in therapy methods.
• Perform evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborate with disciplines to provide patient-centered care.
• Perform other duties as assigned by Director of Rehabilitation.
• Ensure confidentiality, HIPAA, and residents’ rights.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
