Job summary

Apploi in Manalapan, NJ is seeking an Occupational Therapist to provide rehabilitation services at Excelcare in Manalapan. The role focuses on assessing patients, developing treatment plans, and delivering direct care in a long-term care setting.

Responsibilities include setting goals, documenting progress, educating patients and families, and collaborating with the care team. Licensure and registration in the state are required; prior geriatric experience is preferred.

Qualifications

• Must be a licensed Occupational Therapist with current state registration.
• Experience in long-term care and/or geriatric care is preferred.
• Graduation from an accredited OT program is required.

Responsibilities

• Assess referred patients to determine rehabilitation potential.
• Provide direct patient care and evaluate medical condition and functional status.
• Develop and administer a treatment plan with goals aligned to patient capacity.
• Set realistic goals and document patient progress.
• Record patient conditions and progress notes.
• Educate patients and families on therapy methods.
• Complete evaluations, progress notes, re-certifications, and discharge documents.
• Collaborate with the care team to deliver patient-centered treatment.
• Performs other duties as assigned by the Director of Rehabilitation
• Maintain confidentiality and comply with HIPAA and Residents' Rights regulations.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
