Job summary

Excelcare at Manalapan in Manalapan, NJ is seeking a PRN Occupational Therapist to evaluate and treat residents in a long-term care setting. You will assess patients, develop and implement treatment plans, and document progress under physician direction.

The ideal candidate holds an accredited OT degree, NJ license, and has experience in geriatric care. This PRN position offers flexible scheduling within a rehabilitation-focused team.

Qualifications

• Graduated from an accredited OT program.
• Active OT license and state registration required.
• Experience in long-term/geriatric care preferred.

Responsibilities

• Assess referred patients needing rehabilitation services.
• Provide direct patient care to evaluate medical condition and functional capacity.
• Establish and administer a treatment program under physician supervision.
• Set realistic goals according to patient capacity and tolerance.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
• Complete evaluations, weekly progress notes, re-certification and discharge paperwork.
• Collaborate with disciplines to ensure patient-centered care.
• Perform duties as assigned by the Director of Rehabilitation.
• Maintain confidentiality in line with HIPAA and Residents' Rights.

Duties and Responsibilities:

• Responsible for the assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
